Insurance

In-network:

I am currently an in-network provider for BCBS, Cigna, Aetna, Ambetter, TRICARE, United Healthcare/UBH, and UMR. I can file claims for these insurances and your only responsibility is your copay or a payment towards your deductible, due at time of service.

There are some fees that insurance will not cover, such as preparation of documents (for example, FMLA or disability forms for employers, letters, etc.) No show or late cancel fees are also not covered by insurance ($100 per instance). We can discuss these fees if applicable at your first appointment.

Out-of-network:

If you have an insurance plan not listed above, you can call the customer service line on the back of your insurance card to inquire as to your out-of-network benefits. If your plan will allow you to see me as an out-of-network provider, I can provide you with documentation to submit to your insurance so that you may be reimbursed (if applicable). 

Payment

I accept Visa, Mastercard, and Discover card, as well as HSA cards and personal checks or cash. You may also place a credit card on file so that we don't have to spend time in our session with collecting payment.

60 minute intake session: $180

50 minute follow up session: $165
